http response garbled at the start line

Sometimes, the http reponse contains garbled characters at the start line. The following picture is an example:

This error happens randomly. I can not find a way to replay.

My environment:

Ubuntu 12.04 X64 php 5.6.8 phalcon 1.3.4 nginx 1.8.0

I have checked all my files and there are not files encoded with UTF-8 + BOM.
